Scar revision by the pinhole method using a 10,600-nm carbon dioxide laser

This study introduces Pinhole for Burn & Surgical Scar to non-surgically correct poorly oriented scars with inappropriate direction and color differences that occur after trauma or surgery, instead of surgical methods like Z-plasty. A 10,600nm carbon dioxide (CO2) laser is applied in superpulse mode at 2.0W for 3 sessions at 4-8 week intervals. After local anesthesia with EMLA cream, multiple small holes are made in the Scar at 1-3mm intervals, and a hydrocolloid dressing is applied for 1 week. Pinhole for Burn & Surgical Scar induces the regeneration and rearrangement of collagen bundles by creating microscopic holes from the epidermis to the deep dermal layer, thus improving dermal thickness and Scar texture. Post-procedure bleeding and exudation are minimal, and recovery time is short. The authors report that Pinhole for Burn & Surgical Scar can be easily applied to patients with poorly oriented scars by simply changing the operating parameters of the CO2 laser, which is available in most dermatology clinics.
